Mater Aged Placement Service (MAPS) is a care placement and advisory service, based at the Mater hospital, which accepts referrals from Mater Health Services and the wider community.
As part of Mater Health Services, Mater Aged Placement Service promotes the values of Mercy, Dignity, Care, Commitment and Quality.
Our experienced staff have expert knowledge of the health and aged care systems. Working alongside patients and their families, our aim is to simplify the transition into an appropriate facility, and provide professional advice and understanding when settling in.
Assessment of your patient's aged care requirements
Liaison with and referral to Aged Care Assessment Teams (ACAT), who are responsible for assessing eligibility for an aged care placement
Identification of which aged care facilities may be suitable in consultation with the patient and their family
Assistance with often lengthy paperwork, including application and asset forms for up to five aged care facilities
Ongoing communication and liaison with the patient, family and aged care facilities until a suitable aged care placement is secured.
The Mater Aged Placement Services is a private offering and fees will apply for use of this service. As not all placements are the same, patients with more complex needs may require additional services.
