Important information about SmartForm referrals

What has changed?

In order to consolidate and simplify referral avenues for uninsured patients across the state, Mater has ceased the use of Healthlink SmartForms for referrals - effective Friday 12 May 2023.

This change will impact public adult specialist referrals and antenatal referrals sent via SmartForms.

Why has this change occurred?

Mater is dedicated to supporting GP experience, communications and improving the quality of referral information for our clinicians. After a recent review of referral methods, Mater has decided to simplify processes by reducing the number of electronic referral portals and no longer accept SmartForm referrals. This change will improve safety and quality of care for our patients and enable our GPs to use alternative referral methods to reduce referral rework.
