CHaT: Mater’s new discharge summary system

Communication from Hospital at Transfer (CHaT) is Mater’s new discharge summary system.

The new system has been designed to provide our patients and GPs with the information they need to safely transition from hospital care.

The new system was designed by clinicians, for clinicians with and will align us with new national guidelines for discharge summaries and will also be a reliable way to send our discharge summaries to GPs.

Dr Rebecca Dunn (MBBS FRACGP) from Women’s Wellbeing has played an active role in ensuring a GP perspective was maintained through the design of Mater’s new discharge summary. 

In addition, Mater has also leveraged the opinions and feedback obtained through our General Practice Liaison Program, as well as the findings from the discharge summary survey recently distributed conducted by Queensland Health. 

The CHaT system will have:

  • A new report format for patients and GPs
  • One discharge summary per patient journey
  • Visibility of contributions from other clinicians involved in patient care
  • Integration with other Mater systems 
  • Greater utilisation of secure electronic messaging for secure and timely delivery

You may notice these changes when Mater implements CHaT from February 2018.
