Mater at Home can partner with you to check on the welfare and wellbeing of your patients and coordinate care and support if necessary.

Our multidisciplinary home visit team can offer:

• Welfare and wellbeing checks
• Hospital avoidance programs
• Nursing and allied health assessments
• Pathology collection
• Coordination of social supports
• Assistance with coping with isolation
• Food security and nutritional support
• Home safety and falls risk assessments

Following the visit, we will contact you with a status report on your patient and their wellbeing, in addition to discussing any recommendations for support.
