Why advance care planning is particularly important now
Advance care planning (ACP) is a voluntary process that provides people the opportunity to consider their future health care preferences before a crisis occurs. COVID-19 is urging us to think more about the importance of ACP, especially for patients already experiencing illness, frailty or advanced disease. What to do with advance care planning (ACP) documents
Please send all completed copies of Advance Health Directive, Enduring Power of Attorney and Statement of Choices documents to the Office of ACP via post, fax or email.
Received documents are reviewed and uploaded so patients’ documented wishes can be easily accessed when required by public hospital clinicians, including Mater, Queensland Ambulance Service and GPs registered for access. To register for GP access to The Viewer visit www.health.qld.gov.au/hp-portal

The Office of ACP reviews all advance care planning documents received to ensure they are complete and effective before uploaded. Senders of documents are contacted in a timely manner to assist them if issues are identified.
