Antenatal Clinic Referrals

Mater Mothers’ Hospital Antenatal Clinic uses a standard referral form that must be used when referring a public patient to Mater (please note, referral and catchment criteria for maternity care apply). For more on Mater Mothers' catchment infoirmation, click here. 

Please ensure forms are completed in full before presentation back to the antenatal clinic.  Any incomplete referral forms received, will be returned to the referring doctor.

Practice Management software - referral template forms.  Required data fields are able to be auto-completed from your management system.

Please note that prior to importing the template into your practice management system, you will be required to save it as a document to your local or network drive - do not open the document prior to saving. To complete the import, open and follow the instructions attached to the relevant software template below.

Select the practice software you use then fax referral, send e-referral via secure messaging or post completed form.

Private patients

Patients living outside the Mater public maternity catchment may still choose to birth at Mater as a private patient, taking advantage of the private patient facilities and options available for insured or self-funding patients.
