Antenatal clinic referrals

Mater Mothers’ Hospital Antenatal Clinic has developed a new standard referral form. From Monday 19 October 2009 only referrals using the standard referral form will be accepted.

• Mater Mothers’ Hospital Antenatal Clinic will only accept referrals on this form.
• If referrals are received on other referral forms they will be returned to the GP with the correct referral form attached, with a letter of explanation.
• If referrals are incomplete, they will be returned for completion with a letter of explanation.

Mater is moving towards a standard referral for all specialities within its public health service. This standard referral form will eliminate the need to for us to re-contact you for clarity around clinical information. We anticipate this will dramatically improve the speed with which we are able to process referrals to our services.
